Lost Florida Driver License or Lost Florida ID Card

• Proof of identification documents as required by the REAL ID Act of 2005**
• Fee for replacement is $31.25

Stolen Florida Driver License or Stolen Florida ID Card

• Proof of identification documents as required by the REAL ID Act of 2005**
• Fee for replacement Driver License or Identification Card is $6.25 if a police report has been filed

Name Change/Address Change

• Proof of identification documents as required by the REAL ID Act of 2005**
• Must present original or certified copy of name change document
• Must file address change within 10 days after moving
• Fee for replacement Driver License or ID Card is $31.25
