Identification Documents Required

Customers applying for an original Florida driver license, or Florida identification card (ID Card) MUST present proof of identification documents as required by the REAL ID Act of 2005.

Please visit the Division of Driver License Web Site for a listing of the proof of identification documents needed.

NOTE: U.S. birth certificates must be an original or certified copy, from the county or state office in charge of holding such records. Birth certificates from hospitals or other medical institutions are not acceptable.

Only original or certified copies of documents are accepted, and ALL documents presented MUST be of the same name. Names that do not match MUST have a marriage certificate, divorce decree, or other legal name change document(s) ordered by the court, signed by a Judge, and shows the name(s) from birth to current.
